Special Instructions: All autopsies require an ordering MD. Cases require completion of a "Request and Consent for Autopsy" form (available under "Requisitions") be completed and sent to UHN Autopsy. Once the case is accepted, complete medical history and lab/imaging results are required, be it through Connecting Ontario or otherwise.
